Lesson plan template

It's back to school time, and this semester I'm playing both sides of the field. I've got 24 credits of classes I'm taking (yikes!), and I'll be testing the waters teaching at my student-teaching school, too. Since I know I'll be making a ton of lesson plans up for both, I thought I'd create and upload a template for lesson plans to save myself a little time.
